  • Thesis, Dissertation and Project Forms

    Thesis, Project or Dissertation Committee Form

    Required for all students completing a thesis, project or dissertation for the initial appointment or the revision of their committee. 

    Thesis, Project or Dissertation Defense Report Form

    Required for all students who are completing a thesis, project, dissertation.

    Thesis, Project or Dissertation Approval Form 

    Required for all programs to meet degree requirements. This form is used to signify final approval of your thesis, project or dissertation.

  • Continuous Registration Policy

    Continuous registration is expected every semester as appropriate for the program, from admission through graduation, until all requirements for the degree are completed.

    To make continuous progress in a graduate program, students have the following options:

    • Register for at least 1 graduate-level credit applicable to their graduate degree, or
    • Pay the continuous registration fee to remain active in the graduate program although not registered in any courses. Students are also expected to register or pay the continuous registration fee for the summer if they use university facilities or consult with faculty during the summer.

    The continuous registration deadline is the same as the deadline for registration for thesis, research, and independent study courses, i.e., the end of the ninth week of fall and spring semesters or the end of the seventh week of the summer semester.

    Failure to undertake continuous registration may result in previously deferred (DF) grades taken for thesis research becoming permanent grades. Students not making continuous progress or not on an approved leave of absence (see Leave of Absence on this page) may be placed on academic probation (see Academic Standing on this page) or, in some cases, removed from graduate degree-seeking status.   • Advancement to Candidacy

    Some master鈥檚 programs and all doctoral programs require students to apply for advancement to candidacy. Advancement to candidacy status is a prerequisite to graduation and is determined by the program chair or designee. Candidacy is the point in a graduate study program at which the student has demonstrated an ability to master the subject matter and has progressed to the level at which a GSP can be approved. For doctoral program students, an Advancement to Candidacy Form serves as the final GSP.

    To be approved for candidacy, a student must:

    • Be in good academic standing.
    • Demonstrate competence in the methods and techniques of the discipline, which may include passing a comprehensive examination.
    • Receive approval of the independent scholarship, thesis or research project proposal from the student鈥檚 graduate committee.
    • Satisfy all prerequisites, remove all academic deficiencies and satisfy all terms of provisional admission.
    • Submit an approved final official GSP.

    Mechanical Engineering or Project Management MS

    Please see your advisor. Candidacy requirement information is due at least, if not before, the semester in which you intend to graduate. If using an individualized GSP, remember to update your final GSP in DegreeWorks to match your program's candidacy requirements.

    Clinical-Community Psychology PhD

    See PhD program coordinator for information and formsPer the UAA catalog, doctoral students in some programs may not advance to candidacy until they have passed written and/or qualifying exams. Due at least one semester before the semester in which you intend to graduate. Remember to update your GSP in DegreeWorks to match your candidacy form.

    • Request for Temporary Leave of Absence Required to temporarily suspend your studies while admitted to a graduate degree program. If you are not 鈥渃ontinuously registered鈥 (see catalog policy) or submit a Request for Leave of Absence, you will be removed from degree-seeking status and will have to be reinstated before resuming your studies.
    • Graduate Student Employee Workload Exception Form Required when requesting a waiver of the UAA student employment guidelines in allowing a graduate student  who has been awarded an assistantship to work more than 20 hours per week or 40 hours per pay period. International students on F-1 or J-1 visas are not permitted to work more than 20 hours per week and therefore are not eligible for this exception This form in not needed during summer employment. This form is not applicable for GA's having outside employment during their contract which is on violation of their GA employment award letter.
